This is one of two forms you will need if you continued to work and stayed on your or your spouse’s group health coverage at age 65 and also delayed enrolling in Part B of Medicare. You will need to have this form completed by your employer so that you can avoid a late enrollment penalty when you enroll in Part B of Medicare:
Form CMS-L564 – Employer Information Request Form

This is the second of the two forms you will need to enroll in Part B of Medicare after age 65 and not be subject to a late enrollment penalty. Y0u will first need to get Form CMS-L564 completed by your employer. Next, you will complete form CMS-40B and take both of these forms to the local Social Security office. They will then be able to help you get set up for Part B of Medicare:
